Initial Assessment
An initial assessment is required for every client to determine if Hippotherapy (Equine-Assisted PT, OT or ST) is indicated or if Adaptive (Therapeutic) Riding is appropriate. This initial assesment also assist in establishing baseline information.
FOLLOWING IS A BREAKDOWN OF FEES:
$45.00 Initial Assessment
$55.00 > Adaptive (Therapeutic) Riding with an Instructor Per Lesson (paid monthly)
Adaptive Riding requires a Physicians Statement
$85.00 > Hippotherapy with a Licensed Therapist Per Lesson (paid monthly)
Hippotherapy requires a Pysician’s Prescription
and a Physician's Statement

Payment
Payment can be made in cash or check made out to RISE Rehab.
Payment is the responsibility of each client. Some Insurance Companies do cover some of the cost of therapy; however, RISE Rehab does not currently bill insurance directly. Upon request, we can provide a Superbill for you to submit to your Insurance Company.
Cancellations
CLIENT RESPONSIBILITY
When you are accepted into the program...volunteers, horses and staff are assigned and look forward to working with you each week. If you are unable to attend a session, please cancel at least 24 hours prior to session.
RISE REHAB RESPONSIBILITY
The staff at RISE make every effort to provide excellent and professional services. If circumstances ever develop that would prevent riding and unmounted lessons, you will be contacted at the number you designated on your registration form.
Scholarships
Scholarships can be applied for and are awarded based on individual circumstances, financial need and scholarship fund availability. At the end of the awarded scholarship period, a rider can re-apply for continued lessons.
Applications are available upon request.

Guidelines
To protect our clients, volunteers and horses, weight guidelines have been established. These guidelines are relative to the horses and volunteers available and will be addressed on an individual basis.

Dress Code
Shorts and Tank Tops are Prohibited,
No Exceptions!
Helmets Will Be Provided

Illness
For the benefit of everyone, the client should not participate in riding or therapy if there has been vomiting, diarrhea or fever in the last 24 hours.
If cancellations occur due to illness, please contact your instructor as soon as possible, prior to your lesson time.
Conduct
Noise and activity can distract instructors and riders resulting in an unsafe environment for horses. A small cabin is available on-site for the convenience of parents and families to observe lessons from a distance.
RISE Rehab is a No Smoking facility and the use of drugs and alcohol are strictly forbidden. No weapons of any kind are permitted on the premises. We reserve the right to ask anyone to leave the premises.
Registration
The first step to receiving services at RISE Rehab is to fill out our Client Registration Form. Once your registration form has been received, you will receive an email inviting you to establish your Patient Portal account.

It’s our hope that all prospective clients, eligible to participate in the program, will begin as soon as possible. Acceptance depends on availability of volunteers, horses, safety precautions and contraindications.
Waiting lists are utilized when the program is at capacity.
